The attached form is to be used for all user requests to create (add) new Intake Disposition records in SACWIS.
Required entries are identified and represent the minimum information needed for us to complete the request. The form provides drop downs for the data required, where applicable.
Forms that do not contain all of the required data will be returned to the requester for completion.
Please attach Problem Report and completed form to the email when reporting these problems to the Help Desk.
