Process for Submitting Ohio SACWIS Sensitive Data Corrections

Family, Children and Adult Services Procedure Letter No. 197 was published on June 22, 2010, and contains the approved procedure for submitting requests for changes to sensitive data in Ohio SACWIS. 

The following steps provide details on what the Automated Systems Help Desk will require to process requests for data changes.

1.      To identify authorized delegates for submission of data change requests, the Automated Systems Help Desk must have a valid form JFS 01669, “Delegation of Authority for Automated Systems Sensitive Data Corrections”, on file for your agency.  If the agency director elects not to designate delegates, each request for data change will require the director’s signature.

Signed JFS 01669’s should be emailed to the Automated Systems Help Desk in PDF format.  This format is not editable and provides a part of the audit trail

The number of designees is based on county size in relation to CPOE designations.

·         Large metropolitan counties may designate up to six persons.

·         Small to medium counties may designate one or two persons.

2.      For each data change request, the delegate (or agency director for agencies with no delegates), must complete, sign and submit a form JFS 01671, "Automated Systems Sensitive Data Correction Request".

You need JavaScript enabled to view it.  in PDF format.  This format is not editable and provides a part of the audit trail.  Agencies without scanning capability, may fax the JFS 01671 to the Automated Systems Help Desk at 614-728-9678.
