Question:  Are all State/ODJFS forms, letters and/or reports in the system?  If not, why aren't they?  In particular, the ODJFS 1616.

Answer:  The JFS 01616 Form is not maintained in Ohio SACWIS. The decision was made not to maintain the JFS 01616 simply because the form is completed by the biological parents. 

A sampling of the JFS forms/letters/reports that are maintained in Ohio SACWIS can be found in the attachment below.
