This article will guide both public and private agency users on how to access the Ohio Certification for Agencies and Families (OCAF) system.

 

7078 Guide: 

Applicant Information:

Name - Required

Work Phone - Required but may be a cell phone number

County - Not Required

Primary Work Street Address- Required and must include full address including City, State and Zip

Non-state Email Address - Required

Date of Birth - Optional

Cell phone - Optional (can be the same as the work phone)

Work Email address - Required (can be the same as Non-state Email Address)

PW Recovery PIN - Optional

Prior State or County Worker - Not Required

Existing or Previous RACF/JFS ID/OHID: Not Required, should only be filled in if the user has an existing OHID that begins with a '5'

Agency Type: Not Required

Contract Employee: Not Required

 

Access Requested:

ODJFS Network must be checked, all other check boxes not required

OTHER Access - Required - please add the Agency Persona that best matches your role at your agency (list of personas can be found in the Accessing OCAF document above)

Business Role - not required

 

Signatures: 

Applicant Signature*/Date - Required

Supervisor Signature*/Date - Required

*Signatures must be completed in ink as we cannot accept digital signatures
